MAKEUP ALLERGY AWARENESS AND WAIVER

All face brushes, sponges and makeup products are kept sanitary before and during a makeup application. A different clean and sanitized set of brushes will be used for each client. New sponges and disposables are used to apply certain products. Client acknowledges that he/she has or will consult their physician or dermatologist prior to receiving makeup services and will report any skin condition or allergy to the makeup artist/stylist prior to application. If the client has any adverse reaction to any skin, makeup or hair product, the client expressly agrees to seek immediate medical treatment for such adverse reaction. Client further agrees to release the makeup artist/stylist from any liability from all skin complications due to allergic reactions.

PHOTOGRAPHY RELEASE AND WAIVER

I hereby give Yaneth Morales Pro HMUA, its assigns, licensees and legal representatives the irrevocable right to use my name/photograph/image/audio recording/video recording/and likeness (“My Image”) in all forms and manner including but not limited to publication on internet websites, social media advertising, broadcasts and any other publications as released to or by Yaneth Morales Pro HMUA. I understand that Yaneth Morales Pro HMUA and its affiliates cannot control unauthorized use of “My Image” by persons not associated with Yaneth Morales Pro HMUA once “My Image” has been published. I hereby forever waive any right to inspect or approve any publication of “My Image” by Yaneth Morales Pro HMUA. I have carefully reviewed and understand the above provisions and agree to be bound by them.

POLICY ABOUT EQUIPMENT AND MAKEUP & HAIR KIT

Under no circumstances is anyone (including pets) allowed to touch or use the equipment and/or makeup & hair kit owned by Yaneth Morales Pro HMUA. Should any of these items become damaged, contaminated or missing due to the unauthorized use by the client or others around (including pets); the client agrees to replace or pay the original and full cost of all damages.

RISK OF UNINTENTIONAL BURNS AND ACCIDENTAL FALLS

The client accepts that there is a possibility of risk of unintentional burns due to the use of heat tools by Yaneth Morales Pro HMUA or any of its affiliates. In the same way, the client accepts that there is a risk of falls due to the use of electrical extensions in which the client could trip and fall. The client accepts the risk and accepts that no lawsuit will be filed against Yaneth Morales Pro HMUA or any of its affiliates if this accidentally happens. Yaneth Morales Pro HMUA and its affiliates always use a security standard when providing service for their clients. The client's initials below are proof that they agree to the above.
